The Patient Navigation Program strives to improve access to healthcare services by providing patient assistance, reducing access barriers and targeting health disparities.  Patients follow a person-centered care plan and are connected to viable and culturally appropriate resources.

The Housing Resource Program provides transitional housing placement services and wraparound programming. We assist individuals recovering from mental illness and those transitioning from homelessness and addiction.  We also provide a Safe Care level of support for pregnant and postpartum women. Our programming provides a holistic approach that considers the mental health of the individuals and the supportive environment needed to live healthy and independent lives.

An individual’s financial situation often correlates to their feeling of wellbeing or is a cause of stress or anxiety. Our Financial Wellness Program improves financial literacy and, as a result, mental and physical health outcomes. We offer both a core training program and an advanced component for people who are in need of a deeper dive into their financial wellness journey.

Our Maternal Mental Health program provides crucial mental health support and resources for pregnant and postpartum mothers. New mothers and mothers-to-be need a supportive network for their mental wellness to better care for themselves and their babies.
